Gender-Affirming Hair Transplant

What is a gender-affirming hair transplant?

A transgender hair restoration—often referred to as MtF or FtM gender-affirming hair restoration—is a specialised procedure designed to alter a patient’s hairline and hair density to align with their true gender identity.

Are you a suitable candidate?

Patients seeking gender-affirming hair restoration are looking to reshape their hairline or facial hair to better reflect their true gender identity. Hairline shape is one of the most prominent gender markers on the human face: masculine hairlines typically feature higher, flatter, or recessed “M-shaped” temples, whereas feminine hairlines present a lower, rounded, and softer oval frame. Essential requirements for a successful procedure include:

  • Sufficient Donor Capacity: Adequate healthy hair follicles available in your donor area at the back or sides of the head (or secondary body donor zones for facial hair creation).
  • Stable Scalp Health: Healthy scalp tissue that is free from active dermatological conditions.
  • Clear Transition Planning: Alignment with your overall medical transition timeline, including coordination with your hormone replacement therapy (HRT) routine.

During your private consultation at our Glasgow clinic, our doctors will conduct a sensitive, detailed evaluation of your hair density, discuss your aesthetic goals, and collaborate with you to design a bespoke hairline or beard frame that reflects your gender journey.

Elscott Medical Approach

Gender-affirming hair restoration demands an intuitive understanding of facial sexual dimorphism, surgical precision, and deep clinical empathy. At Elscott Medical, we recognise that your hair restoration is a deeply personal step in your transition. Whether soft-rounding an M-shaped hairline for a transfeminine patient or constructing a dense, structured beard and moustache for a transmasculine patient, our approach is bespoke and empathetic.

The procedure

On the day of your procedure at our Glasgow clinic, your doctor will hand-draw your new hairline or facial hair contour. Under gentle local anaesthetic, individual follicular units are harvested from the permanent donor zone using refined Follicular Unit Excision (FUE) micro-tools. The harvested grafts are immediately sorted and preserved under high-power microscopy to isolate delicate single-hair units essential for soft, natural borders.

Using ultra-fine implanter devices, your surgeon creates recipient sites tailored to your desired gender markers. For feminine hairline lowering, single grafts are hand-implanted at flat, natural angles to create a soft, micro-irregular edge that looks completely natural. For transmasculine patients, we map facial hair growth patterns to build authentic, strong jawlines and moustaches. The procedure typically takes between four to eight hours depending on graft count, during which you relax in complete comfort and privacy.

Recovery and results

Most patients returning to light daily activities within 5 to 7 days. Micro-crusts and mild redness form around the newly implanted follicles, naturally shedding within the first week as the skin settles. You will receive a bespoke post-procedure aftercare kit and direct surgical guidance to protect your grafts and support clean, comfortable healing.

The transplanted hairs naturally shed within two to four weeks post-procedure as part of the standard biological reset phase. Brand-new, permanent growth begins to emerge around month three or four, steadily building in density, thickness, and soft coverage. Final mature results settle between 10 to 14 months, delivering a permanent, beautifully framed hairline or facial hair structure that aligns authentically with your identity.

Frequently Asked Questions

Hormone replacement therapy plays a significant role in hair characteristics. For transfeminine patients, estrogen and anti-androgens help suppress DHT, halting further male pattern baldness and often stabilizing or slightly thickening native hair. For transmasculine patients, testosterone increases body and facial hair growth over time. We generally recommend being on a stable HRT regimen for at least 12 months prior to surgery. This allows your hormone levels and native hair patterns to settle, ensuring your surgeon can design a transplant that complements your long-term medical transition.

In many cases, yes. A tailored FUE hair transplant can dramatically feminise the upper face by lowering a high forehead, filling in recessed temporal corners, and creating a soft, rounded, oval hairline frame. However, if you have prominent underlying bony structures (such as a heavy brow ridge) and plan to undergo Facial Feminisation Surgery (FFS) involving forehead contouring, we strongly advise completing your FFS before having a hair transplant. This ensures the newly transplanted hair is not disturbed or displaced by facial surgical incisions.

Absolutely. While testosterone therapy often stimulates natural facial hair growth, some transmasculine individuals experience patchy growth or limited cheek and moustache density due to genetics. Using Beard & Moustache FUE Transplants, we can harvest healthy hair follicles from your scalp donor area and carefully implant them into your cheeklines, jawline, goatee, or moustache. The transplanted hairs will integrate permanently, allowing you to grow, trim, and style a full, natural beard.

Unconditionally. We provide an inclusive, safe, and deeply respectful environment for every patient. Your medical history, transition background, personal details, and chosen pronouns are treated with strict medical confidentiality and dignity by our surgical and clinical team.

Prices very depending on the specifics of each case but, in general, Gender-Affirming Hair Restoration costs between £3,000 and £7,000.
